Preparing for Your First Appointment

To assist us in providing you with the best possible care, please bring the following with you on the day of your appointment:

A referral slip from your doctor. A list of medications you are presently taking. Complete insurance information including the member's name, social security number, date of birth and name of the employer.

Please arrive 10-15 minutes early for your first appointment to complete the registration form. The registration form is also available online. To expedite your first visit, you can print the form and complete it prior to coming to our office (please go to our Download Forms section). Minor children under the age of 18 years must be accompanied by a parent or legal guardian.
