WELCOME TO OUR OFFICE
We are here to make your visit with us as comfortable as possible. Our goal is to provide you with the best possible care in a safe, relaxed, compassionate environment. We will do our best to make you feel relaxed, comfortable, and cared for.
Your first appointment with us will usually be for a consultation with Dr. Karras. At this first visit, Dr. Karras will examine you and review your x-rays and medical history. He will then discuss with you your diagnosis and treatment options. Minor procedures can often be performed on the same day. However, a complicated treatment or medical history may make it necessary to schedule your procedure for a separate appointment time, after your initial evaluation has been completed.
PREPARING FOR YOUR APPOINTMENT
To assist us in providing you with the best possible care, please bring the following with you on the day of your appointment:
A referral slip from your doctor.
A list of medications you are presently taking.
Complete insurance information including the member's name, social security number, date of birth and name of the employer.
Please arrive 10-15 minutes early for your first appointment to complete the registration form. The registration form is also available online. To expedite your first visit, you can print the form and complete it prior to coming to our office (please go to our Download Forms section). Minor children under the age of 18 years must be accompanied by a parent or legal guardian.
IF YOU ARE HAVING GENERAL ANESTHESIA OR SEDATION
If you will be put to sleep (general anesthesia or sedation) for your surgery, please observe the following: 1) Do not eat or drink anything for 8 hours prior to surgery. However, if you are taking prescription medication, you may take it at your scheduled time with a small sip of water. 2) You must be accompanied to the office by a responsible adult driver. 3) Wear loose, comfortable clothing with short sleeves. 4) Do not wear contact lenses.
FEES, INSURANCE AND PAYMENTS
We strive to provide you with the highest quality care at a reasonable cost. Payment for services is requested at the time services are received unless other financial arrangements have been made in advance. We will gladly submit claims and work with your insurance carrier to maximize your benefits. However, please be aware that you are personally responsible for all fees charged by our office. It is your responsibility to provide us with correct and up-to-date insurance information and to verify network participation. If you are a member of a dental HMO or PPO in which our office participates, you must bring any required referral with you on the day of your appointment. Most insurance plans will reimburse you a certain percentage of the actual fees after the deductible amount has been satisfied. For your convenience, we accept personal checks, cash and most major credit cards. We also offer interest-free payment plans through Care Credit, a healthcare financing company. This requires a simple application that can be completed in our office or online at www.carecredit.com.
If you have any questions about our financial policies, please feel free to call us at 847.677.6647.
